Best Airline + program to earn miles?

Question for the frequent travellers amongst you guys, whats:

A) the best airline you’ve ever flown with (and why)?
B) the best program to subscribe to to earn Miles and hence get free/cheap upgrades, lounge access, etc?

I take about 10-15 flights a year so this info would be very helpful!
